For couples, I would not start by asking which Osaka hotel looks the most romantic. I would start with a harder question: how many evenings will you actually give the hotel? If three of four nights end after dinner, bars or late shopping in Namba, a skyline, lounge or beautiful shared space may be something you pay for but barely use. If one or two evenings are deliberately left open, the same premium can become worthwhile.
My rule is simple: choose the area by where your nights end, then choose the hotel by what you will still want after you return. If you have not settled Namba versus Umeda yet, use Where to Stay in Osaka for First-Time Visitors or Namba vs Umeda first. A couples-hotel shortlist only makes sense after the base itself is right.

My first filter: count usable hotel hours, not stars
A hotel feature only has value when your itinerary leaves time to consume it. I would therefore mark every evening before comparing rates: late outside, hotel evening, early night or day-trip recovery. If a premium feature is realistically used once across four nights, treat the whole four-night premium as the cost of that one use.
For example, if an upgraded room were hypothetically ¥8,000 more per night for four nights but you used the lounge properly only once, you would effectively be paying ¥32,000 for that one lounge evening. That does not automatically make it bad value — the evening may matter to you — but it stops the room upgrade from hiding behind a small-looking nightly difference. This is the calculation I would make before calling any couples hotel “worth the premium.”
Centara Grand: choose it when the hotel is part of the date, not just where the date ends
Centara Grand Hotel Osaka is the clearest choice here when one evening genuinely belongs to the hotel. The current Deluxe Miyabi Club Floor is 27–30 m², with city views and complimentary Club Lounge access; the Corner Miyabi Club room is 40 m² and the Suite Miyabi Club is 56 m². The important fact is not simply that these categories are larger. The decision is whether you will return early enough for the view, lounge and room to become part of the evening.
I would pay for that layer when the plan contains a deliberate reset: come back before the night is over, change, have a drink, watch the skyline and stay in. I would not pay for it simply because this is a couple trip. If you are in Dotonbori until late on most nights, I would rather buy the lower-friction location or put the room premium into one meal or one deliberately upgraded night. voco Osaka Central: strongest when “stay in” does not mean “stay in the bedroom”
voco Osaka Central works for a different reason. Its current Premium rooms are 30–31 m² versus 21–26 m² for Standard rooms, while LOKAL HOUSE on the first floor combines an Italian restaurant with a café and bar. I see that as an evening-continuity product: return, put the bags down, have a drink, eat downstairs and end the night without another station or taxi decision.
For me, voco’s value is continuity: enough room for two people to reset, then dinner and drinks without another cross-city move.
The trade-off is location. If three nights out of four still finish around Namba or Shinsaibashi, the quieter Kyomachibori/Higobashi base can turn into repeated late-night friction. In that itinerary I would not let design photography overrule the map. But if your plan already includes one hotel dinner, one early night before Kyoto and a couple of evenings that do not depend on Minami nightlife, the slower base starts making sense. Mitsui Garden Hotel Osaka Premier: choose it when recovery is the evening
Mitsui Garden Hotel Osaka Premier has a 16th-floor public bath open from 15:00 until 09:00 the following morning. That makes it unusually forgiving for a couple who returns late from a long day: the feature still exists when the sightseeing is over. My reason to choose it is therefore not “romance” in the conventional sense. It is that the hotel gives the day a clean ending.

This only becomes a couples advantage when both travellers actually value the bath; otherwise the feature is mostly irrelevant to the pair.
This is also the easiest recommendation to reverse. If one of you dislikes communal bathing, I would remove most of the bath value immediately. If your evenings repeatedly finish in Umeda or Namba and you resent an extra final leg, I would also downgrade it. A hotel should remove a recurring problem; it should not make you hurry back because you feel guilty about unused facilities. Zentis Osaka: choose shared-space value only if the stay is long enough to repeat it
Zentis Osaka is the one I would consider when two people need the hotel to function beyond the bedroom. The current Studio is 25 m² and Corner Studio 32 m². More importantly, Room 001 provides self-service laundry and dryers, ironing, beverages, a microwave and a small library, while the hotel also has a separate guest lounge. The official access information puts JR Osaka Station about a 12-minute walk away from the Sakurabashi Exit, so this is not the option I would choose merely to minimise the final walk.

Zentis shifts some of the value out of the room: that is useful on a longer stay, but easy to overpay for on a short, packed itinerary.
For three nights, I would be sceptical unless you already know you will use those spaces. For five nights, the answer can reverse: doing laundry can reduce what you pack, the lounge gives you somewhere to sit without treating the bed as a sofa, and a quieter base can become more valuable after several intense days. The point is not that long stays automatically justify Zentis. The point is that repeated use finally gives the shared spaces a chance to earn their cost. Three nights and five nights should produce different answers
On a three-night first trip, I would weight the final transfer and the place where dinner ends more heavily than most hotel facilities. There are too few evenings to recover the value of several premium features. If two nights already end in Dotonbori, I would usually solve that geography before paying for a lounge or bath elsewhere.
At five nights, repeated hotel life starts to matter. Two people need more space to unpack, repack, sit, dry clothes and occasionally stop moving. That can push voco or Zentis upward. Centara can still be the right answer if one night is deliberately protected for the hotel. Mitsui Garden becomes stronger if the bath genuinely enters your nightly routine. The longer stay does not make the more expensive property better; it simply increases the number of times a useful feature can pay you back.
Failure test: would you still choose this hotel if the signature feature disappeared?
This is the red-team test I would use before paying. Remove the skyline from Centara, LOKAL HOUSE from voco, the public bath from Mitsui Garden or Room 001 from Zentis. If you would still happily stay there because the location and room already fit, the upgrade is probably optional. If the whole case collapses without that one feature, then you need to be unusually sure that your itinerary leaves time to use it.
Also test the worst evening, not the best one: heavy rain, two shopping bags, a 22:30 return, sore feet after Kyoto, or one person wanting to go straight to bed. A couples hotel should still work on that night. If its value depends on both of you being energetic, dressed up and back early, the recommendation is fragile.
When I would ignore all four
If USJ is the centre of the trip, I would solve Universal City access before romance. If you are making repeated JR day trips to Kyoto, Kobe and Himeji, I would strongly consider staying immediately around Osaka Station. If you have a very early KIX departure, airport routing can outrank atmosphere. And if most evenings end in Dotonbori, a simpler Namba hotel can be a better couples hotel than any property on this list because it returns time and energy every night.
Verdict: buy the evening you will actually use
My default shortlist is conditional. Choose Centara Grand when one evening belongs to the skyline and lounge. Choose voco Osaka Central when you want a larger room plus dinner and drinks without another cross-city move. Choose Mitsui Garden Hotel Osaka Premier when the best ending is bath then bed. Choose Zentis Osaka when a four- or five-night stay makes laundry and shared space repeatable. If your schedule contradicts the reason for choosing the hotel, I would reject the hotel — even if it looks more romantic in photos.
